My first orgasm occurred as an affect of a dream I had when I was 12ish.
I remember vividly that I was dreaming of being alone in a room. I was viewing myself from a 3rd person standpoint. I was in the center of the room, on a metal exam table. I was lit but the room all around me was dark. I couldn't see walls or doors.
A doctor entered the room, in a lab-coat, holding a clipboard. But then he was satan; a dark creature composed of shadows and malicious intent. He had sex with me on the table. Though it was unexpected and even a little awkward, I found it incredibly arousing to watch.
I awoke from the dream quickly as my very first orgasm rippled through my body. I remember the fluttery feelings of the contractions tickling my insides and pulling me out of the dream. I lay awake for a moment, confused but feeling quite nice.
I think it's interesting to note that at this point in my sexual maturity, my sub-conscious equated climax with the character who defines ultimate evil in my culture.

a variant form of the ‘vase with a lid’.
